There should be some way to differentiate between a man wanting to be a woman and vice versa. Maybe when mentioning their name there should be a + for a man wanting to be a woman and an - for a woman wanting to be a man before their names. Agreed?

Very few places have Freedom of Speech like we do. Some ability to censor what people can say exists in almost every other nation. You can even go to jail for saying "mean things" in Britain! As for the Japanese, culturally they would object to using a non-political public platform (this case the Olympics) to make a political statement.
