They are separate features within the ESPN app. To watch ESPNU on the app, you must have a subscription to a cable/satellite/internet TV provider which contains ESPNU.

You cannot watch ESPNU with an ESPN+ subscription. An ESPN+ subscription only gives you access to games that are being broadcast on ESPN+.
You have to have a TV provider in order to watch ESPN, ESPN2, and ESPNU.
